RN School Nurse Care Coordinator - Part Time

SUMMARY:
The RN school nurse Care Coordinator’s primary responsibility is to support the management of the school population to promote effective education of staff, students and families related to health care to promote learning. This will involve developing and monitoring care management processes and training school staff in the delivery of proper health care to students. It will also include identifying the high acuity student population and working to ensure care coordination for these students related to procuring doctor’s orders, creating individual health care plans and emergency plans for educators. The RN will coordinate the care for these students by collaborating with physicians, school staff and families. This position will also involve direct student care via technology to the schools as well as assisting the School Health Director with school implementations as needed.
ESSENTIAL FUNCTIONS:
- Consults with school administrators to determine goals and services.
- Serves as a team member in identifying, evaluating, and providing services for children.
- Collects information about the health and developmental status of the student in a systematic and continuous manner.
- Uses data collected about health and developmental status of the student to determine a nursing diagnosis (eCare school nurse utilizes electronic communication, equipment, and UAP assistance in the nursing assessment and care of students).
- Collaborates with physicians, providers, school staff and families in identifying appropriate care management needs for students.
- Develops and maintains individual health care plans and emergency plans with specific goals and interventions that are unique to that student’s needs.
- Develops relationships with high acuity population students and families as an integral member of the education team.
- Assesses barriers when student has unmet medical needs that are affecting their education plan and addresses barriers by collaborating with physicians, school staff and families.
- Serves as a liaison between the school and community health and welfare agencies and the medical profession.
- Provides leadership in the prevention of illness, disability, and the early detection and correction of health problems.
- Assists with the development and maintenance of student health records, including immunizations.
- Coordinates building health day screenings and physical exams.
- Communicates with colleagues, parents, and students.
- Facilitates referral of students and families to appropriate district and community services. Communicates plan of care to school staff, student’s families, and others, as appropriate.
- Participates in ongoing professional growth.
- Provides care for students with scheduled and unscheduled health needs such as administering medications, treatments, and procedures as directed by the student’s physician and District policy.
- Trains and supervises staff in delegated tasks.
- Provides health education for students, staff, and parents.
- Maintains and assures confidentiality of information.
- Participates in professional and/or community organizations.
- Complies with school district policies and operating procedures.
REQUIRED EDUCATION and/or EXPERIENCE:
- Bachelor’s degree in nursing preferred.
- One to three years’ experience in the health care field, preferable community health, pediatrics and/or mental health preferred.
REQUIRED CERTIFICATION, LICENSURE, and/or REGISTRATIONS:
- Licensed as a Registered Nurse with one unrestricted state license is required.
- RN licensure required within the states that eSchool is providing service within 120 days of employment.
