Hours/FTE: 72 hours/2 weeks, 7 on and 7 off
Reports To: Director of Pharmacy Operations
SUMMARY:
Provides pharmaceutical care, which includes all medication related activities provided directly to the patient with definite outcomes intended to improve the patient’s quality of life. The Pharmacist must accept personal responsibility and accountability for the patient’s outcomes.
ESSENTIAL FUNCTIONS:
- Reviews prescriber medication orders for safety and efficacy. Approves medication for dispensing to patients at contracted facilities utilizing multiple EMRs.
- Evaluates medication therapy of patients and makes recommendations to physicians and other prescribers to provide the best possible therapy for the patient in the most efficient manner while maintaining ASHP standards of pharmaceutical care.
- Provides drug information to physicians, nurses and other healthcare professionals.
- Able to communicate with other healthcare professionals in an effective manner via telephone, written communication, or other means, if necessary.
- Demonstrates knowledge of age-specific factors and performs satisfactorily related to patient’s age neonate through geriatrics as noted on competencies.
- Counsels patients on the proper usage, effects and side effects of medication.
- Responsible for accurate documentation of all work completed within the order management system.
- Maintains customer-service focus to achieve excellent working relationship with the remote contracted facilities.
REQUIRED EDUCATION and/or EXPERIENCE:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree required.
- Hospital experience or completion of ASHP-accredited Residency preferred.
REQUIRED CERTIFICATION, LICENSURE, and/or REGISTRATIONS:
- Pharmacist licensure in the residing state is required.
- The following state Pharmacist licensure will be required based off job assignments TX, NE, OK, MT, and NV.
