Nurse Practitioner- Senior Care- Remote
General Hours of Work: 7 days a week (Day and Night Coverage Required)

Reports To: Director- Clinic Operations
SUMMARY:
The responsibility for the provision of medical care to patients in Avel eCare Senior Care under the direction and supervision of a physician. Nurse Practitioners practice under the rules and regulations of the Nurse Practice Act of the states in which the practitioner is providing services.
ESSENTIAL FUNCTIONS:
- Evaluates the physical, mental, psychosocial, and developmental needs of patients; obtains complete patient history; performs physical examinations using techniques of auscultation, observation, and inspection; orders laboratory and radiologic studies and interprets test results; carries out prescription drug orders; orders treatments for acute and chronic conditions as appropriate; manages common acute or chronic illness and referrals via two-way video technology; collaborates with the interdisciplinary team, primary care providers, and specialists physicians as needed; formulates and communicates treatment plans with patients and families; provide education to assist with patient healthcare needs and decisions.
- Uses the appropriate diagnostic tests in accordance with protocols and procedures approved by the appropriate licensing and administrative authorities.
- Documents the processes of care delivery.
- Coordinates the medical and nursing plan of care to ensure patient-centered care.
- Meets with the physician for consultation and collaboration as required. Assesses, diagnoses, and treats patients according to acceptable standards of practice.
- Assists with the development and implementation of new Avel eCare Senior Care services and programs, including quality improvement projects, protocol development, workflow mapping, and clinical effectiveness research.
- Assists with the design, revision, and implementation of protocols.
- Recognizes and reacts appropriately to environmental safety factors related to patient care.
- Participates in data review and analysis of Avel eCare Senior Care operational outcomes and metrics as requested.
- Interacts with Avel eCare Senior Care customers to facilitate the development of best practice standards and professional relationships ensuring a strong relationship between Avel eCare Senior Care and its customers. Provides education to customers to support high quality care.
REQUIRED EDUCATION and/or EXPERIENCE:
- Graduate of an accredited Nurse Practitioner program required.
- Master’s or Doctorate degree required.
- One or more years of experience preferred.
- Geriatrics, internal medicine, nursing home, and/or experience with geriatric populations preferred.
REQUIRED CERTIFICATION, LICENSURE, and/or REGISTRATIONS:
- Licensed as a nurse practitioner with one unrestricted state license is required.
- Licensed as a Registered Nurse with one unrestricted state license is required.
- An active Federal DEA (Drug Enforcement Administration) and State CSR certification (if applicable) is required.
- APRN (advanced practice registered nurses) licensure required within the states that Avel eCare Senior Care services are being provided within 6 months of hire including any state specific education requirements.
- Federal DEA and/or State DEA certification may be required within the states that Avel eCare Senior Care services are being provided within 6 months of hire.
- Completion of state specific certification/education within the initial and renewal time periods is required.
- Must maintain full and current certification as a nurse practitioner from a nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.
